Humeral head: medial epicondyle. Ulnar head: olecranon and posterior border of the ulna.
Insertion
Pisiform, then via the pisohamate and pisometacarpal ligaments to the hook of the hamate and the base of the fifth metacarpal.
Action
Flexes and adducts (ulnar-deviates) the wrist; stabilises the pisiform for the hypothenar muscles.
Nerve supply
Ulnar nerve (C7–T1).
Blood supply
Ulnar artery.
Clinical relevance
The ulnar nerve runs between its two heads and grooves between it and the flexor digitorum profundus — cubital tunnel release frees it here.
