Origin

Nuchal ligament and spinous processes of C7–T4.

Insertion

Mastoid process and lateral superior nuchal line.

Action

Extends and ipsilaterally rotates the head — the 'bandage' muscle of the posterior neck.

Nerve supply

Dorsal rami of the middle cervical nerves.

Blood supply

Occipital artery.

Clinical relevance

Its trigger points refer pain to the vertex; splenius spasm accompanies whiplash injury.
