Posterior tubercles of C2–C7.
Insertion
Upper surface of the first rib behind the subclavian groove.
Action
Side-bends and stabilises the cervical spine; the largest scalene elevates the first rib.
Nerve supply
Anterior rami of C3–C7.
Blood supply
Ascending cervical artery.
Clinical relevance
Together with the anterior scalene it anchors the thoracic outlet's muscular boundaries; fibrous bands beneath it throttle the lower plexus roots.
