Interosseous membrane, posterior tibia and fibula.
Insertion
Navicular tuberosity, all cuneiforms, cuboid and metatarsal bases 2–4.
Action
Inverts the foot and plantarflexes the ankle; the foot's key arch support — its tendon sling holds the medial arch like a stirrup.
Nerve supply
Tibial nerve (L4–L5).
Blood supply
Posterior tibial and peroneal arteries.
Clinical relevance
Posterior tibial tendon dysfunction is the commonest cause of adult acquired flatfoot; the posterior tibial pulse is felt just behind its tendon at the medial malleolus.
