Tibialis anterior

Location: Leg

Paired structure: one on the left and one on the right side of the body.

Origin

Lateral condyle and upper lateral shaft of the tibia, interosseous membrane.

Insertion

Medial cuneiform and first metatarsal bases (plantar surface).

Action

Dorsiflexes the ankle to clear the toes in the swing phase of gait, and inverts the foot; eccentrically lowers the foot after heel strike — controlling the foot-slap.

Nerve supply

Deep peroneal (fibular) nerve (L4–L5).

Blood supply

Anterior tibial artery.

Clinical relevance

Deep peroneal palsy or L5 radiculopathy drops the foot; acute anterior compartment syndrome after tibial fractures makes passive toe extension excruciating — a surgical emergency.
