Origin

Rami of the ischium.

Insertion

Central perineal body (with bulbospongiosus fibres).

Action

Stabilise the perineal body — the keystone of pelvic floor support and the anchor for anal and urethral sphincters.

Nerve supply

Pudendal nerve (S2–S4).

Blood supply

Perineal artery.

Clinical relevance

The perineal body they weave is the surgical target of external anal sphincter repair; obstetric tears here (third/fourth degree) demand systematic reconstruction.
